The North Eastman Health Association Diabetes Education Resource has been in existence since 1987. A Primary Health Care Nurse Educator and Registered Dietitian are available in six areas in our region for people with diabetes, to provide information about this disease. These teams of diabetes educators travel to many communities in North Eastman on a regular basis. Please contact the team in your area for more information.

In 2004, The North Eastman Health Association - Diabetes Education Resource partnered with MB Telehealth to pilot a unique approach to providing local and timely access to information about diabetes including advice on monitoring, diet, medication and active living. Karen and Sharlene were the cutting edge leaders in making this website possible.

Your Appointment

You will be contacted for an appointment with the Diabetes Education Resource team. You will have the opportunity to speak with the Nurse Educator and the Registered Dietitian. In order to be prepared for your visit, see "What to Expect" below.

  What to Expect

1) Bring a 3 day Food Record of your typical eating habits. You can use the downloadable Food Record Sheets or make your own. The best thing to do is write down everything that you eat and drink from the time you wake up in the morning until the time you go to sleep. This will make it easier to discuss your current eating habits and find out where changes could be made to improve your diet. Also, make a note if it is a special occasion, such as a birthday party.

Follow this link to the instructions for completing your Food Record and an Easy Downloadable Food Record

2) Bring a list of the medications you are currently taking. Use the downloadable Medication Record Sheet to list them.

3) Bring the Blood Glucose Machine you are using, so that it can be tested for accuracy.

4) Bring your questions with you. It can help to write them down, so that you can be sure they will be answered.

5) Bring a friend! Two sets of ears can be better than one. A lot of information is discussed at your appointments.

A follow-up appointment can be booked for you to review your progress and to answer any questions you may have. We are looking forward to meeting you!
